A.1 – Application to Rent Use this form to obtain a prospective tenant’s information for evaluation. This form was last updated in October 2023 to comply with the Investigative Consumer Reporting Agencies Act and requires that users (property owners) complete the last page, “Notice Regarding Background Investigation Pursuant to California Law.” Also, this form includes a section for use of “Reusable Tenant Screening Reports” now mandatory to accept in West Hollywood (currently optional for rental housing providers in areas other than West Hollywood and potentially other jurisdictions in the future).REV 2.11.24”
A.2 – Deposit Receipt Use this form when a tenant applicant provides a deposit prior to signing of a lease.
A.3 – Inventory Checklist – Premises Only Use this form to document the condition of the rental unit at the time of move-in and later when a tenant moves-out. This form will assist you in documenting any deductions from a tenant’s security deposit.
A.4 – Notice of Negative Credit Information and Rental Application Rejection Use this form to notify a tenant applicant of negative credit information and rejection of rental application.
A.5 – Pet Application Use this form when tenant applicant has a pet that will live in the rental unit.
A.6 – Rental Unit Holding Deposit Agreement

Use this form when accepting a deposit payment to hold rental unit off of the market pending tenant applicant screening.
A.7 – Screening Fee Agreement Use this form when accepting payment for credit check and background verification (tenant screening).
A.8 – Landlord Verification Form Use this form to send to your tenant applicant’s prior landlord to verify information such as on-time rent payments, etc.  The form must be signed by your prospective tenant to authorize the previous landlord to provide the information you are requesting.
A.9 – Notice of Tenant Credit Reporting (S.B. 1157) for Owners of 15 or More Assisted Housing Development Units This form applies to owners of 15 or more “Assisted Housing Development” Units.  On or after July 1, 2021 and until July 1, 2025 for new leases, and by October 1, 2021 for leases in existence as of July 1, 2021, landlords of one or more assisted housing developments (total of 15 or more units among all properties) must now offer tenants to have their rental payments reported to at least one consumer credit reporting agency.  Landlords are required to offer tenants the option to have their rental payments reported annually after the initial offer if tenants do not initially elect to have their rent payments reported.

Landlords may charge any tenants that elects to have rent reported the lesser of $10.00 per month or the actual cost to the landlord to provide the service.   If the fee remains unpaid for 30 days or more, the landlord may stop reporting the tenant’s rental payments and the tenant shall be unable to elect rent reporting again for a period of six months from the date on which the fee first became due.

A tenant who elects to have rent reported may file a written request to stop that reporting, provided that, the tenant is then prohibited from electing rent reporting again for at least 6 months thereafter.  A tenant who elects to have rent reported does not forfeit specified tenant rights and if a tenant makes deductions from rent or otherwise withholds it, as authorized under the law, the deductions or withholding does not constitute a late rental payment.

An “Assisted Housing Development” means a multifamily rental housing development that receives governmental assistance under any of the programs set forth at: www.leginfo.legislature.ca.gov/sec.65863.10

Senate Bill 1157 Update: California Passes Legislation to Help Renters Establish Credit for Paying Rent

A.10 – Rental Criteria for Rental Applicants It is important that you establish your rental criteria and apply it consistently to all applicants you deal with.  Appropriate rental criteria can also standardize and simplify your tenant selection process and will help you narrow down your minimum requirements as a landlord ranging from rental history and background checks to pet and parking policies.  Establishing and following a rental criterion protects both the landlord and the tenant and is a critical part of a thorough tenant selection process.  THIS SUGGESTED RENTAL CRITERIA MAY NOT ADDRESS ELEMENTS YOU MAY WANT OR NEED FOR YOURSELF.  YOU MAY USE THIS DOCUMENT AS A GUIDE IN SETTING CRITERION THAT MEETS YOUR SPECIFIC REQUIREMENTS.

B.1 – Security Deposit Refund Form Use this form to calculate the amount of security deposit to be refunded to a tenant. Attn. City of Los Angeles property owners: be sure to calculate interest due at published rates.
B.2 – Standard Security Deposit Agreement Use this form to provide a tenant with the terms governing receipt and refund of a security deposit, and for tenant to acknowledge receipt of “house rules” and lack of security at premises, among other things.
B.3 – City of Los Angeles Bulletin: Interest on Security Deposits This explains how much interest you should pay on security deposits each year between 1990 and 2020 for rental units subject to the Rent Stabilization Ordinance in the City of Los Angeles. Landlords are required to pay (1) simple interest using the rates in this Bulletin, or (2) actual interest earned on security deposit funds deposited in your bank account.

B.4 – Standard Notice of right To Request Initial Inspection Use this form when an initial inspection of the rental unit is requested by a tenant. This form documents the condition of a rental unit at the time possession is first accepted by renter which may then be helpful later when inspecting condition of a rental unit upon move-out and documenting deductions from tenant’s security deposit.>

D.9 – Electric Vehicle Charging Station Agreement Use this form if a tenant wishes to install an electric vehicle charging station at the tenant’s cost.
D.10 – House Rules: Obligations of a Resident, Rules and Regulations Addendum to Lease and Rental Agreement * Use this form to have a tenant acknowledge tenant obligations and house rules at your property.
D.11 – Lead Booklet Before renting pre-1978 housing, owners must disclose the presence of known lead-based paint hazards in the housing premises. At the time of lease signing, renters must receive this federally approved pamphlet on lead poisoning prevention.rev.9.21.23

D.16 – Political Sign Addendum Use this form to set forth the rules for the placement of political signage at a property.
D.17 – Pool and Hot Tub Addendum Use this form when a pool and/or hot tub is located on the property.
D.18 – Proposition 65 Warning Addendum Proposition 65 requires warnings to be provided to each known adult occupant of a rental property at the time of renting, leasing, or “hiring out” the property. The warning must be initially made and renewed each year by one of the following methods: (1) delivering a letter to each known adult tenant; (2) sending an electronic message to each known adult tenant’s email address; or (3) including the warning in a lease or rental agreement. In this Addendum, the name of at least one chemical that is a carcinogen and/or reproductive toxicant that is present at the property must be listed. Although landlords are not required to use this addendum and warn residents of the presence of cancer-causing chemicals, use of this form provides a “safe harbor.” Declining to provide the warning may pose a risk of enforcement action, whereby landlords would need to defend their warnings as being “clear and reasonable.” Landlords that fail to provide adequate warnings are subject to penalties as high as $2,500 per violation per day.
D.19 – Special Flood Hazard Area Addendum Use this form when your rental property is located within a special flood hazard area.
D.20 – Standard Satellite Dish and Antenna Addendum Use this form when a tenant requests to install a satellite dish and/or antenna at the rental property
D.21 – Water Submetering Addendum Use this form to bill a tenant for water service separately from the rent when the premises are equipped with a water submeter.
D.22 – Information on Mold and Dampness for Renters in California

Effective January 1, 2022, the booklet, “Information on Dampness and Mold for Renters in California” from the California Department of Public Health must be provided to renters for all new tenancies.  This booklet is required by legislation that was passed in 2001 (Senate Bill 732, the “Toxic Mold Protection Act”) but not developed by the Department of Public Health until recently.  It is available in both English and Spanish.
D.23 – lease addendum City of Cudahy Use this form as an addendum to any lease agreement along with Form D.23.1, “Notice of Tenants’ Rights,” to comply with the City of Cudahy’s Ordinance No. 736, Chapter 5.13, “Rent Stabilization” pursuant to the city’s municipal code.This form was updated in 2024 to include the required Notice of Tenant Rights. Property owners are required to provide written notice of the ordinance and tenants’ rights at: (i) Commencement of a lease, or (ii) Renewal of lease. The notice shall be written in the language that the Owner and Tenant used to negotiate the terms of the Tenancy (e.g., Spanish, Chinese, Tagalog, Vietnamese and Korean), as well as English and in a form prescribed by the City

E.2 – Declaration of Service of Notice Use this form when providing any 3-day, 30-day or 60-day notice to a tenant. This form is for your records only.
E.3 – Culver City Notice of Change in Terms of Tenancy Use this form to document changes in terms under a lease in the Culver City (e.g., change in rental rate) and notify tenant of mediation services available through City of Culver City Landlord Tenant Mediation Board.
E.4 – Glendale Notice of Change in Terms of Tenancy Use this form to document any changes in terms under a lease in the City of Glendale (e.g., change in rental rate) and to have tenant acknowledge receipt of offer to enter into a one-year lease.
E.5 – Long Beach Notice of Change in Terms of Tenancy Use this form to document any changes in terms under a lease in Long Beach (e.g., change in rental rate).
E.6 – Los Angeles (City) Notice of Change in Terms of Tenancy For City of Los Angeles only.  Use this form to document any changes in terms under a lease in the City of Los Angeles (e.g., change in rental rate, pass-through of SCEP and registration fees, etc.).  Include copy of Registration Certificate for current year with this notice. Beginning on January 1, 2022, the new annual SCEP Fee is $67.94 per unit – 50% only may be passed-through to renters monthly at $2.83 per unit per month.
E.7 – Los Angeles County (Unincorporated Areas) Los Angeles County Rent Stabilization and Tenant Protection Ordinance (RSTPO), Rental property owners of fully and partially rent stabilized units located in unincorporated areas of the County must provide their tenants with a Notice of Tenant Rights. This form must be provided in all of the following circumstances: (i) When entering into a rental agreement by including a copy of the notice as an exhibit or attachment to the written rental agreement; (ii) When renewing a rental agreement; and (iii) When providing notice of a rent increase or decrease in a fully covered rental unit or a reduction in housing service. In addition, this Notice of Tenant Rights must be posted in a conspicuous location, such as a lobby of the property or common areas (e.g., near mailboxes, in laundry rooms, or at the entrance to the property). E.8 – Notice of Belief of Abandonment Form A, use this form in conjunction with Forms B & C below. Use this form when it appears a tenant has left property behind in the rental unit or appears to have abandoned the rental unit (e.g., moved out).
E.9 – Notice of Right to Claim Abandoned Property (Valued at Less than $700) Form B, use this form in conjunction with Forms A & C. Use this form to notify a tenant of any property that was abandoned by tenant that is worth less than $700, and the process for recovering or disposing of the property if not claimed.
E.10 – Notice of Right to Claim Abandoned Property (Valued at More than $700) Form C, use this form in conjunction with Forms A & B above. Use this form to notify a tenant of any property that was abandoned by tenant that is worth more than $700, and the process for recovering or disposing of the property if not claimed.
E.11 – Notice of Change of Terms in Tenancy (Non-Local Rent Control & State Mandated AB 1482 Rent Control) Use this form to document any changes in terms under a lease in rental units not subject to a local jurisdiction’s rent control and for units subject to state mandated rent control per Assembly Bill 1482. (e.g., change in rental rate).
E.12 – Notice of Owner / Agent Office Hours Use this form to notify tenants the days and times you wish to be contacted for non-emergency matters.
E.13 – Reminder: Your Rent is Past Due Use this form to provide a tenant with a “friendly reminder” when rent is past due. Use this form prior to providing tenant with a “Three Day Notice to Pay or Quit.”
E.14 – Santa Monica Notice of Change in Terms of Tenancy. Revised 6.23.23 Use this form to document any changes in terms under a lease in the City of Santa Monica (e.g., change in rental rate). For periods beginning on September 1st and ending on August 31st.
E.15 – Sixty (60) Day Notice to Quit Use this form to notice a tenant to return possession of premises within 60-days for specific reasons such as failure to enter into new lease; move-in of owner or the owner’s spouse, children, parents, or resident manager; or for a unit that is not otherwise subject to eviction restrictions.
E.16 – Thirty (30) Day Notice to Quit (rev 4.10.23) Use this form to notice a tenant to return possession of premises within 30-days for specific reasons such as failure to enter into new lease; move-in of owner or the owner’s spouse, children, parents, or resident manager; or for a unit that is not otherwise subject to eviction restrictions. In addition, use this form to notify a tenant to return possession for unauthorized pets or occupants in unincorporated areas of Los Angeles County, but only through and including January 31, 2024 at which time thereafter, you may again serve a “3-Day Notice to Perform or Quit” for these two types of lease breaches.
E.17 – Thirty (30) Day Notice to Pay Dishonored Check Use this form to notice a tenant to “make good” on a check payment within 30-days.
E.18 – Three Day Notice to Pay or Quit Use this form to notice a tenant to pay rent within 3-days, or upon failure to do so, to return possession of premises within 3-days.

Attn. City of Los Angeles Rental Housing Providers: Effective January 27, 2023, any written notice terminating a tenancy for a tenant at-fault legal reason must be filed with the Los Angeles Housing Department (LAHD) within three (3) business days of service on the tenant per Los Angeles Municipal Code 151.09.G.9 & 165.05.B.5. At-fault eviction notices can be filed at https://housing.lacity.org/eviction-notices. Attn. Santa Monica Property Owners: Landlords are asked to email to the Santa Monica City Attorney’s Office (CAO) at unredacted copies of any notices of any endeavors to evict (pandemic-related or not), including a termination (pay or quit) notice or a summons and complaint for unlawful detainer, within two days of serving such notice or summons and complaint on a tenant. According to the City’s ordinance; however, violations of this requirement do not result in an affirmative defense for the tenant in an unlawful detainer action.

E.19 – Three Day Notice to Perform or Quit rev 4-25-23 Use this form to notice a tenant to cease specific behavior or cease performing specific acts within 3-days, such as causing a disturbance or prohibiting owner access to tenant’s unit, or upon failure to do so, to return possession of premises within 3-days. rev 4-25-23
E.20 – Three Day Notice to Quit Use this form to notice a tenant for breach or other lease violation that cannot be cured (such as conducting criminal activity or creating a nuisance at premises).

E.25 – Notice of Change of Terms in Tenancy (Non-Rent Control and Misc. Other Jurisdictions) Use this form to document any changes in terms under a lease in rental units not subject to a local jurisdiction’s rent control or state-mandated rent control per Assembly Bill 1482. (e.g., change in rental rate). In addition, use this form to document changes in terms for miscellaneous jurisdictions listed at the top of the form.
E.26 – Sixty (60) Day Notice to Quit (No-Fault Eviction for Use Under A.B. 1482 State Rent Control) Descr.: Senate Bill 567 amended the “no-fault” tenancy termination rules under Assembly Bill 1482, which is also known as the “California Tenant Protection Act of 2019.” Under the new rules, which become effective on April 1, 2024, no-fault eviction notice and disclosure requirements were expanded and additional requirements imposed upon property owners wishing to terminate a tenancy for no-fault reasons. Use this form to provide a “60-Day Notice to Quit” for residential rental units covered under state rent control (e.g., Assembly Bill 1482) for residential rental units covered under Assembly Bill 1482 and when stricter local regulations covering no-fault tenancy terminations do not exist.

E.28 – Los Angeles (County) Notice of Change in Terms of Tenancy Use this form to document any changes in terms under a lease in the County of Los Angeles (e.g., change in rental rate, pass-through of registration fees). This form should be used in unincorporated areas of Los Angeles County, particularly rental units subject to the County’s rent stabilization ordinance (RSO). This form may also be used for non-RSO properties; however, pass-through of the rental registration fee is not applicable.When entering into a new rental agreement renewing a rental agreement; or providing notice of a rent increase or decrease or notice of reduction in housing service, housing providers must also provide a Notice of Tenant Rights (See Form E.7).
E.29 – Culver City Rent Control Ordinance Notice All Culver City landlords subject to the City’s Rent Stabilization Ordinance must post the Rent Control Ordinance Notice in a conspicuous location in the common area, at the entry or entries to the building or units, or other similar location or locations as necessary to provide tenants a reasonable opportunity to view the notice.
E.30 – Oxnard Three (3) Day Notice to Perform or Quit REV 4-25-23 Use this form for properties located within the City of Oxnard to notice a tenant to cease specific behavior or cease performing specific acts within 3-days, such as causing a disturbance or prohibiting owner access to tenant’s unit, or upon failure to do so, to return possession of premises within 3-days.  This three (3) day notice contains additional language required by Oxnard City code. REV 4-25-23
E.31 – Oxnard Sixty (60) Day Notice to Quit (No-Fault Eviction) Use this form for properties within the City of Oxnard to notice a tenant to return possession of premises within 60-days for specific reasons such as failure to enter into new lease; or “no-fault” reasons such as move-in of owner or the owner’s spouse, children, parents, or resident manager; or for a unit that is not otherwise subject to eviction restrictions.  This Sixty (60) day notice contains additional language required by Oxnard City code.(REV 1.30.24)
E.32 – Oxnard Change in Terms (Rent Increase) Use this form to notify tenants in the City of Oxnard of a rent increase (allowable rent increase up to 4% per annum).  This is a City of Oxnard provided form.
E.33 – City of Los Angeles Declaration to Evict for Owner Move-In Complete this form and file with the City of Los Angeles’ Housing Department (LAHD) at the address on the form for a no-fault eviction resulting from owner occupancy. The address for filing is: Los Angeles Housing Department, Post Office Box 17100, Los Angeles, CA 90057. E.34 – City of Los Angeles Declaration to Evict for Resident Manager Move-In Complete this form and file with the City of Los Angeles’ Housing Department (LAHD) at the address on the form for a no-fault eviction resulting from resident manager move-in. E.35 – City of Los Angeles Declaration to Evict for Declaration of Intent to Evict for Alleged Drug-Related Nuisance, Illegal Drug Activity, or Gang-Related Crime Complete this form and file with the City of Los Angeles’ Housing Department (LAHD) at the address on the form for an eviction resulting from Declaration of Intent to Evict for Alleged Drug-Related Nuisance, Illegal Drug Activity, or Gang-Related Crime. The address for filing is: Los Angeles Housing Department, Post Office Box 17100, Los Angeles, CA 90057. The form may also be uploaded on the City of L.A.’s website at https://lahd.service-now.com/efs.
E.36 – Notice to Enter Premises Use this form to notify tenant of intent to enter rented premises for inspections, repairs, etc.
E.37 – 30-Day Notice to Pay or Quit Per CARES Act In the event rental property meets specified conditions, this form should be used in lieu of a “Three Day Notice to Pay Or Quit” form. This form should be used at any rental properties where the renter is past due on rent, and: (i) Any tenancy at the rental property is or was subject to either a Section 8 or United States Department of Agriculture (USDA) Housing Choice voucher, (ii) The rental property is subject to a federal program, including Section 8 and/or Low Income Housing Tax Credit (LIHTC) housing programs, and/or (iii) The rental property is secured by a mortgage issued or guaranteed by: Fannie Mae or Freddie Mac, Federal Housing Administration (FHA), Veterans Administration, a USDA direct loan or USDA guaranteed loan.) The Federal Violence Against Women Act (VAWA) (2013) requires a written notice of an applicant’s or tenant’s rights under the statute. VAWA requires covered housing providers to distribute the notice, among other times, with a notice of eviction. The notice must be accompanied by the federal agency-approved certification form and must be available in non-English languages for persons with limited English proficiency. The appropriate notices and federal agency approved certification should be obtained on the U.S. Housing and Urban Development website.”.

H.24 – Santa Monica: Notice of Residential Leasing Requirements Santa Monica Municipal Code Chapter 6.22 requires all landlords to provide this notice to all prospective tenants.  The City of Santa Monica imposes the following requirements for all residential leases: (i) lease must be in writing and the initial lease term must be no less than one year; (ii) the rental unit must be leased unfurnished; and (iii) tenant must use and occupy the rental housing unit as their primary residence.  Landlords are required to provide this notice to their tenants in both English and Spanish – this form is in 2 pages to accommodate both languages. The notice must be provided both when you give your prospective tenant a rental application and when you provide any written lease to be signed. If you are providing the application or lease electronically, the notice must also be provided electronically. Within 60-days of the commencement of the lease, your tenant must provide you with at least two (2) documents to prove the rental unit is their residence (see requirements stated on the notice).

**This notice must be provided to tenants who as of JULY 1, 2021 had not paid one or more rent payments between MARCH 1, 2020 and THEREAFTER and such rent amounts are still due and owing. This notice must be provided to all such tenants on or before JULY 31, 2021.**

In the event rent is due for periods prior to October 1, 2021, then continue to utilize the Fifteen (15) Day Notice to Pay or Quit for rent owed during periods prior to October 1, 2021 (See Fifteen-Day Notice forms at K.3 and K.4).  Note: In the event your tenant has declared COVID related financial impacts and has not remitted at least the 25% of rent due on or before September 30, 2021 per Assembly Bill 832, then you utilize the Fifteen (15) Day Notice to Pay or Quit for all rent owed during periods prior to October 1, 2021 (See Fifteen-Day Notice forms at K.3 and K.4).

Proposition 65 Warning Addendum (D.18) Proposition 65 requires warnings to be provided to each known adult occupant of a rental property at the time of renting, leasing, or “hiring out” the property. The warning must be initially made and renewed each year by one of the following methods: (1) delivering a letter to each known adult tenant; (2) sending an electronic message to each known adult tenant’s email address; or (3) including the warning in a lease or rental agreement. In this Addendum, the name of at least one chemical that is a carcinogen and/or reproductive toxicant that is present at the property must be listed. Although landlords are not required to use this addendum and warn residents of the presence of cancer-causing chemicals, use of this form provides a “safe harbor.” Declining to provide the warning may pose a risk of enforcement action, whereby landlords would need to defend their warnings as being “clear and reasonable.” Landlords that fail to provide adequate warnings are subject to penalties as high as $2,500 per violation per day.
Reminder: Your Rent is Past Due (E.13) Use this form to provide a tenant with a “friendly reminder” when rent is past due. Use this form prior to providing tenant with a “Three Day Notice to Pay or Quit.”

Santa Monica: Notice of Residential Leasing Requirements (H.24) Santa Monica Municipal Code Chapter 6.22 requires all landlords to provide this notice to all prospective tenants.  The City of Santa Monica imposes the following requirements for all residential leases: (i) lease must be in writing and the initial lease term must be no less than one year; (ii) the rental unit must be leased unfurnished; and (iii) tenant must use and occupy the rental housing unit as their primary residence.  Landlords are required to provide this notice to their tenants in both English and Spanish – this form is in 2 pages to accommodate both languages. The notice must be provided both when you give your prospective tenant a rental application and when you provide any written lease to be signed. If you are providing the application or lease electronically, the notice must also be provided electronically. Within 60-days of the commencement of the lease, your tenant must provide you with at least two (2) documents to prove the rental unit is their residence (see requirements stated on the notice).

Thirty (30) Day Notice to Pay or Quit Per CARES Act (E.37) In the event rental property meets specified conditions, this form should be used in lieu of a “Three Day Notice to Pay Or Quit” form. This form should be used at any rental properties where the renter is past due on rent, and: (i) Any tenancy at the rental property is or was subject to either a Section 8 or United States Department of Agriculture (USDA) Housing Choice voucher, (ii) The rental property is subject to a federal program, including Section 8 and/or Low Income Housing Tax Credit (LIHTC) housing programs, and/or (iii) The rental property is secured by a mortgage issued or guaranteed by: Fannie Mae or Freddie Mac, Federal Housing Administration (FHA), Veterans Administration, a USDA direct loan or USDA guaranteed loan.). The Federal Violence Against Women Act (VAWA) (2013) requires a written notice of an applicant’s or tenant’s rights under the statute. VAWA requires covered housing providers to distribute the notice, among other times, with a notice of eviction. The notice must be accompanied by the federal agency-approved certification form and must be available in non-English languages for persons with limited English proficiency. The appropriate notices and federal agency approved certification should be obtained on the U.S. Housing and Urban Development website.
Sixty (60) Day Notice to Quit (E.15) Use this form to notice a tenant to return possession of premises within 60-days for specific reasons such as failure to enter into new lease; move-in of owner or the owner’s spouse, children, parents, or resident manager; or for a unit that is not otherwise subject to eviction restrictions.
Sixty (60) Day Notice to Quit (No-Fault Eviction for Use Under A.B. 1482 State Rent Control) (E.26) Descr.: Senate Bill 567 amended the “no-fault” tenancy termination rules under Assembly Bill 1482, which is also known as the “California Tenant Protection Act of 2019.” Under the new rules, which become effective on April 1, 2024, no-fault eviction notice and disclosure requirements were expanded and additional requirements imposed upon property owners wishing to terminate a tenancy for no-fault reasons. Use this form to provide a “60-Day Notice to Quit” for residential rental units covered under state rent control (e.g., Assembly Bill 1482) for residential rental units covered under Assembly Bill 1482 and when stricter local regulations covering no-fault tenancy terminations do not exist.

Standard Lease Up to Twelve (12) Month Rent Deferral Addendum (12-Month Deferral Jurisdictions) (J.13) (For Any Jurisdiction Allowing Up to 12-Month Rent Deferrals Only) Use this Addendum to document deferred rental amounts and UP TO 12-MONTH repayment plan due to COVID-19 (Coronavirus) impacts.
